The corona pandemic has caught many psychotherapists unprepared.

The restrictions in our daily lives such as curfews, restraining orders, decree on working from home, but also closed schools, nursery schools and care facilities – all this has left its marks in our society. In the pandemic, many adults are suffering from anxiety disorders, existential fears and depressions. But the pandemic has also made itself felt on children and youths. The evaluation of several studies indicates that frequency of psychological symptoms has increased. For a large number of girls and boys, disorders requiring treatment were detectable, for example showing in symptoms relating to an attention deficit/hyperactivity disorder or temper tantrums and outbursts of fury, i.e. symptoms of an oppositional behaviour disorder. This has led to a substantially increased demand for psychotherapeutic treatments.

Significant increase in patient requests

A survey conducted by the DPtV (German association of psychotherapists) showed that last year an average of 4.9 requests for therapy were made every week. This number has already increased to 6.9 requests this year, and the proportion of psychotherapists who have received more than ten patient requests per week has even doubled. However, the survey shows that only one out of four patients requesting therapy receives an appointment for an initial interview. The consultation of new patients by phone, as requested by the DPtV for some time now, is refused by the National Association of Statutory Health Insurance Physicians (KBV).

The situation still is as it was: Only patients with at least one appointment in the practice in the six quarters before the consultation are considered as “known patients” and can make use of the option for telephone consultation. An acute treatment without personal contact, for instance via psychotherapeutic video sessions, is therefore almost rendered impossible. A situation that is equally afflicting for both patients and therapists.

High corona infection risk during psychotherapeutic sessions

But there is something that troubles many therapists even more. The large flow of patients in practices inevitably increases the therapists’ personal risk of a corona infection. Since psychotherapeutic sessions take place in a face-to-face situation, the risk that each party will infect the other is especially high. Even if a patient is vaccinated, recovered or has a negative test result – the risk of an infection literally is “in the air”.

Numerous scientific studies show that, merely by breathing and mainly through talking, aerosol particles carrying the SARS-CoV-2-virus already spread across several metres and can float in the room air infectiously for several hours. Even though wearing a mouth nose mask may offer reliable protection against larger droplets, it does not fully prevent the inhalation of these tiny aerosol particles. Therefore, during a therapy session of approx. 40 minutes in a poorly ventilated room it is almost impossible to protect you from inhaling air that has been inhaled and exhaled again by the person facing you – and which thus might be contaminated with infectious coronaviruses. To make matters worse, the breaks between the tightly scheduled treatment appointments in most cases are so short that there is only little time to to ventilate the room sufficiently after a session. As a result, potential virus particles remain in the room air and increase with every therapy session.

Another good reason using mobile air cleaners is the personal well-being during the treatment. Undisturbed communication between the therapist and patient is the centre of every therapy session. Therefore it is absolutely necessary to reduce sounds of noise, so as to prevent negative impacts on the atmosphere. Hence, the idea of open windows to supply the room with fresh air is only practicable to a limited extent. Ventilation in the oncoming winter proves even more difficult – with outdoor temperatures around the freezing point.
